When applying for HCBS Waivers or Intermediate Care Facilities for Individuals with Intellectual Disabilities (ICF/IID), the application packet includes the following:

Home and Community-Based Waivers (DD & Mi Via) and ICF/IID Application Form
HIPAA Information – Understanding privacy rights for applicants.
Pre-Service Intake Bureau (PSIB) Contact List – Regional contact details for submitting applications.
Pre-Service Intake Bureau (PSIB) Fact Sheet – Key information about services and eligibility.

Applicants must complete and submit these forms along with supporting documentation to be considered for services.

How to Submit Your Application

1️⃣ Download the Application Form

2️⃣ Fill Out the Application Completely

  • Provide all personal details, including the applicant’s Social Security Number (SSN).
  • Include supporting documentation that confirms a diagnosis of a developmental disability (DD).

3️⃣ Attach Required Documents

  • Neuropsychological/Psychological Evaluations
  • Educational Evaluations & Individualized Education Plans (IEPs)
  • Medical Diagnosis Reports

  • Mail or Fax your application to the address listed for your region on pages 7 and 8 of the application packet.
  • The application date will be the date the Pre-Service Intake Bureau (PSIB) of DDSD receives your completed, signed application.

5️⃣ Connect with Your Pre-Service Specialist

  • After submitting your application, a Pre-Service Specialist from DDSD will contact you to:
    • Review your application.
    • Discuss the next steps.

What Happens After Submitting the Application?

Once your completed application is received, DDSD will:

Confirm Receipt – Your assigned Pre-Service Specialist will review the application.
Assess Eligibility – DDSD will determine whether you meet the definition of developmental disability in New Mexico.
Provide a Decision – You will receive a letter confirming whether you have been:

  • Placed on the Wait List (Yes Match Letter).
  • Asked for Additional Documentation to complete eligibility.

If placed on the Wait List, you may still qualify for alternative services, such as:

  • State General Funds (SGF) – Limited temporary services while awaiting full waiver approval.
  • Centennial Care Community Benefits – Additional Medicaid-based support.
